    Respiratory depression is a widely recognized possible complication of sedation and analgesic administration. Ventilation monitoring using capnography has been determined to be the earliest indicator of hypoventilation and episodes of no-breath. This lecture will discuss ventilation monitoring using capnography during procedural sedation. The activity will:-

    • Demonstrate how to utilize capnography to continuously monitor ventilation during procedural sedation

    • Review the concepts and common terms associated with capnography, as well as the physiology surrounding this application

    • Discuss moderate and deep sedation, how capnography is utilized to monitor both, and why it is integral in the care of the sedated patient

    • Review current Joint Commission regulations

    • Review of common abnormal waveforms associated with sedation and the appropriate interventions

    The purpose of this educational activity is to provide an overview of capnography monitoring in the management of critically ill patients. This activity includes:-

    • Overview of capnography

    • Review of the physiology surrounding its application, and a discussion of normal EtCO2 values to include what conditions cause the values to increase and decrease.

    • Review the difference between EtCO2 and PaCO2

    • A brief discussion of the different capnography technologies

    • Monitoring in different hospital environments

    • A discussion on nursing and collaborative interventions

    During the past few years, organizations such as the Institute for Safe Medication Practices (ISMP) and the Anesthesia Patient Safety Foundation (APSF) have indicated a need to increase patient safety for patients receiving opioids by monitoring ventilation. Although pulse oximetry is known to monitor oxygenation it does not deliver information on the adequacy of ventilation. Capnography has been determined to be the fastest indicator of ventilatory depression. This lecture describes ventilation monitoring using capnography to detect hypoventilation in patients receiving PCA or neuraxial opioids in the postoperative period.
